Insurance Coverage

Most private insurance plans cover massage therapy. Check your plan for details and bring in your physicians referral if your plan requires one. We direct bill to all of the following companies and will with any other company that will allow massage therapy to be directly billed.

If you will be coming in without the primary insurance holder, please print off the appropriate form below along with the Assignment of Benefits form and have them sign both forms.
